Process Automation

Workflows allow you to automate standard internal procedures and processes to save time across your organization. A workflow rule, is the main container for a set of workflow instructions.  Workflows allow to create tasks, email alerts, field updates, and send a secure, configurable API message (in XML format) to a designated listener.  An approval process, automates how records are approved in Salesforce. An approval process specifies each step of approval, including who to request approval from and what to do at each point of the process. Other tool is the process builder.  The Process Builder is a workflow tool that helps you easily automate your business processes by providing a powerful and user-friendly graphical representation of your process as you build it.  And the Cloud Flow Designer lets you design flows without writing any code.

Providers Office & Patient Profile & Demographics

Considered one of the main sources of data because it contains all the information related to patients, providers, vendors, specialty care offices, etc. Here is where we are going to find information segregated by record types and in turn will help you know which type of record you are viewing.


Patient Profile & Demographics

This data object,  contains all the information related to the patients such as name, contract number, address, telephones, email, communication barriers.  The patient profile will allow the physicians and the medical group capture and/or enter relevant data such as:


  • Patient Problem List: Contains the list of all the patients issues that require attention from the physician or the care provider.
  • Patient Family Medical History: It has all the illnesses that siblings and family members, have had, either deceased or alive. This is important for the physician, in order to find out if the patient could possibly have also any of those illnesses.
  • Allergy History: Here we will find the list of the patient allergies and possible reactions.
  • Tobacco Use: Provides a detail of the tobacco usage of the patient and details the amount of cigarettes, start date, etc.
  • Alcohol Use: Determines whether the patient drinks alcohol or not, as well as how much alcohol, type of alcohol and how often does the patient drink.
  • Recreational Drugs: Provides to the physician a clear view of the drugs that the patient uses, if any.
  • Immunizations / Vaccines: Contains the detail of all the immunizations or vaccines that the patient has had along the time.
  • Medication List: Provides the list of medications that the patient is using or has used along the time.
  • Previous Surgeries: Contains the list of any previous surgery that the patient has had in his life. This is here with the purpose of being able to consult it when the patient comes in for a problem that could be related to a previous surgery.

Admission Records

Find relevant information when a patient has been admitted to the hospital. Information like the Admission Date, Admitting physician, room number and some others will be found in this form.  The admission record can register other relevant information such as:

  • Patient Survey:  Brief survey that the patient takes so that we can evaluate the type of service that the patient is getting.
  • Admission Lab Results: Here we will find the detail of the Lab Results gotten when the patient was admitted, only if required. This information will be linked specifically to the admission record.
  • Secondary Diagnosis: Here we will find the record of any other illness that the patient currently has, and that may or may not contribute with the main Diagnosis.
  • Level of Care: Here we will have the intensity of the medical care that will be provided to the patient by the physician or care facility.
  • Clinical Consult: In case that a clinical consult for the patient is needed, it will be detailed in this object. Information like the requested date and consult type will need to be entered to fill out the record.
  • Clinical Procedure: Here we will have the detail of the procedure used to apply to the patient. Information like the procedure date and the CPT will need to be filled out in order to complete the form.Type your paragraph here.

Patient Quality Measures

Here we will list all the measures that comply with the 5 star rating. It is divided between Part C and Part D measures and it is associated with HEDIS Measures.  This modules allows the medical group and the physician understand the HEDIS metrics that are open and available per patient and document.


Lab Encounters and Results

Here we will detail the lab encounters along with the CPT and type of labs performed.  Lab encounters will only have the detail, the remaining information will be placed in the Lab Results object.  Lab Results will provide the results for the related to the lab encounters. Information like the procedure, test units, test ID type and some other information will need to be filled out to complete the form. The data related to these objects will be provided through an alliance and direct connectivity with DHS LabConnect and MisResultadospr.com Inc.


Order Entry

Here we will list the order that the physician has requested, in case something special is needed for the patient. It can be a wheel chair, a gel mattress, air pads, etc. The order entry system designed to allow the physician, to submit special orders to the medical group and other medical group contracted suppliers.


Preauthorizations

Provides the details of the patient, and the reasons why the patient needs to be treated. Here we will have the ICD information and when the procedure will be done.  This module allow the physician, specialist and the medical group to track the procedures that need certain authorizations at the health plan.


Vital Signs

In case of transition care, the nurse that is assigned to the patient and will visit at home, will take all the vital signs for the patient. It has body temperature, blood pressure, body weight, body mass index, etc. metrics.


Care Plans

A list of questions related to certain types of illnesses, to find out how the patient is dealing with the illness, as well well if help is needed or medication needs to be changed because of any side effects.  Currently, questionnaires available are congestive heart failure, diabetes, high blood pressure, and myocardial infarction.  Other questionnaires will be developed based on the need of the customer.


Appeals and Grievances

Contains the information of the patient´s response to something that he or she did not like while on treatment on the medical facility. It can also have a petition for something that the patient needs in order to feel comfortable while staying in the medical facility.


Transportation Arrangements

In case that the patient is transferred from another country to get medical treatments, it is important to know how the patient will be transported between health care facilities.  It also applies when the medical facility provides the travel arrangements to the patient.


ICD & CPT Tables

Stands for International Classification of Disease and here we are going to have all the existent ICD´s so we can associate them with the appropriate records. Stands for Current Procedural Terminology and here we are going to have all the existent CPT´s so we can associate them with the appropriate records. The medical group or insurance company must provide the associated table.
